State of Washington Vehicle Collision Report

Any operator or owner of a vehicle involved in a vehicle collision where there was an injury or property damage of $700 or more is required to complete this form and mail to the Washington State Patrol.

Request a Copy of a Vehicle Collision Report

If you have been involved in a vehicle collision and have submitted your collision report to the WSP you can get a copy of your report with an online request to WSP.
